Asthma action plan
is a plan that is focused to handle a severe
asthma attack condition. Asthma is a unique disease, as its severity
keep changes. Usually when patient’s condition decrease, the asthma will

asthma patients, and must be brought everywhere they go.
Asthma action plan is
related to severity level of asthma, so that you will have a guidance to
control your asthma. Or in a more simple way, asthma action plan would
be your manual book

Writing Your Own Asthma Action Plan

How To Use Your Asthma Action Plan ?

Important data :
Fill in patient name, doctor's name and reachable phone number in case
of emergency case (mobile phone is preferable). Patient name data will
be useful if the patient has difficult talking symptom and is alone in a
public area. This way, anyone can help the patient to contact his/her
doctor or call for ambulance. You need also to fill in the hospital and
ambulance address and phone numbers.
'My Best Peak Flow Performance' column :
Fill in here the peak flow rate of your best performance. This rate will
be useful to determine in which asthma zone you are.
'My Asthma Trigger/Inducer' column :
Break downs of most common asthma trigger and allergen. Mark one or more
of your trigger/allergen, but if it is not available on the list,
describe it in 'other' section. This information will be useful for your
doctor or medical staff to decide the most suitable medication to cure
your asthma.
Asthma Zone (green-yellow-red) :
This section contains the most important data both for patient and the
doctor.
For patient : fill in 'my peak flow meter rate' using the formula for
each zone, based on your best performance rate that you have entered in
'Important data'. When you have asthma symptoms, you can check in which
zone you are, and what action to do. Therefore, learn all the symptoms
and actions in each zone from the beginning, so that you will know what
to do when the asthma attacks.
For doctor : The 'medication-dose-time' columns MUST be filled in with
your doctor supervision, do not fill it in by yourself!
